Your First Visit
In the event that you are visiting us for the first time, we kindly request that you download and fill out the new patient forms beforehand to accelerate the registration process. Additionally, please ensure that you arrive 15 minutes earlier than your scheduled appointment time to verify other mandatory documents, such as photo identification, insurance papers, etc.

(All forms need to be read and signed before your first visit)
During your initial visit, we will allocate approximately an hour for the cleaning, depending on the condition of your dental health. Our team will take the time to get to know you and understand your dental history, as well as any concerns you may have. To develop a comprehensive treatment plan, Dr. Jun and his team will conduct a thorough dental examination and take necessary dental x-rays. You will have ample time to inquire about any questions you may have, and we will present and explain all treatment options and recommendations. In some cases, complex treatment plans may require additional appointments to ensure thorough preparation and presentation.
During your follow-up visits, we will ask you to confirm your current contact details, health records, and insurance status to ensure that we have the most accurate and up-to-date information.
Appointments
We operate on an appointment-based system and kindly ask that you schedule your visit in advance so that we can reserve a specific time for you. We make every effort to adhere to our scheduled appointments and kindly request that you extend the same consideration to us. We respect your time and understand the importance of uninterrupted procedures, as each reserved appointment slot is allocated exclusively for you. If for any reason you are unable to attend your appointment, we kindly request that you inform us at least 48 hours in advance to avoid any cancellation fees. This will also enable us to offer the appointment slot to another patient in need.
We welcome PPO dental insurance plans but do not accept HMO, DHMO, or ePPO plans.
